SAMPLE REPORT — FICTIONAL COMPANY, FICTIONAL SYSTEM

Sample Full AI Act Assessment Report

This is a shortened example of the paid report, written for an invented company so you can see exactly what the GBP 349 buys before you pay. Your own report is generated on screen for your system as soon as payment completes, ready to print or save as a PDF. The free classifier stays free, and this sample is free with no signup.

1. Risk classification memo (excerpt)

System: AcmeMatch, by Acme Decisions Ltd — software that scores and ranks job applicants' CVs for employer customers, who use the ranking to decide who is interviewed.

Classification: High Risk. AcmeMatch falls under Annex III, point 4(b) of the EU AI Act (recruitment and selection of natural persons), so it is high risk under Article 6(2). It is not a prohibited practice under Article 5: it does not score citizens, infer emotions, or exploit vulnerabilities. It is not limited or minimal risk, because its output materially influences access to employment.

Roles: Acme Decisions Ltd is the provider. Each employer customer is a deployer, with its own duties under Article 26 — the register below separates the two.

2. Obligations register (excerpt — five of the full register rows)

ArticleDutyWhat it means for AcmeMatch
Article 9Risk management systemEstablish and document a continuous risk management process for the screening model, reviewed at every model update.
Article 10Data and data governanceDocument training and validation data provenance, representativeness for the candidate population, and bias examination results.
Article 13Transparency to deployersShip instructions for use covering capabilities, limitations, accuracy metrics and known failure modes.
Article 14Human oversightNo automatic rejection: a trained reviewer must be able to inspect the score rationale and override it before any candidate is rejected.
Article 26Deployer obligationsCustomer employers must inform candidates that AI screening is in use, keep logs for at least six months, and monitor operation.

High risk obligations apply from 2 August 2026. The paid report carries the complete register for your tier, each row with its enforcement date and the evidence an auditor will ask for.

3. Annex IV technical documentation skeleton (headings)

  1. 1
General description of the AI system and its intended purpose
  2. 2
Elements of the system and the process for its development
  3. 3
Monitoring, functioning and control of the AI system
  4. 4
    Appropriateness of the performance metrics
  5. 5
Detailed description of the risk management system
  6. 6
Description of relevant changes made to the system over its lifecycle
  7. 7
List of the harmonised standards and specifications applied
  8. 8
    EU declaration of conformity
  9. 9
Post market monitoring system and plan

In the paid report each heading is drafted from your system description, with the gaps you still have to fill marked as such — nothing is invented on your behalf.

4. 30 day compliance action plan (excerpt)

Week 1 — Stop the gaps

Freeze automatic rejections, switch on human review of every adverse decision, and add the candidate notice the transparency duties require.

Week 2 — Evidence the data

Assemble the data governance file: sources, labelling rules, representativeness checks and the bias examination for the current model version.

Week 3 — Write the file

Complete the Annex IV skeleton into a full technical file, draft the instructions for use, and set the logging and record keeping defaults.

Week 4 — Prepare to register

Run the conformity self assessment against the obligations register, close or schedule every gap, and prepare the EU database registration entry.
